I agree to stay within Miranda Westfield facility whilst my child/children are cared for by Mumma’s Little Secret.
I understand that Mumma’s Little Secret can supervise my child/children for up to two (2) hours. Parents/guardians must collect their child/children Mumma’s Little Secret at conclusion of their stay.
I acknowledge that I will be required to pay late fees in addition to the hourly child minding fees if I am late to pick up my child/children. Please refer to our Collection of Children Policy and our Fee policy for further details.
I agree to take my child/children to the bathroom/change nappy before dropping my child/children off at Mumma’s Little Secret.
I understand that Mumma’s Little Secret staff will do their absolute best to settle my child/children. However, if my child/children are unable to be settled within 20 minutes of being dropped off, Mumma’s Little Secret staff will call me and ask me to return to Mumma’s Little Secret to settle my child/children. If my child/children will still not settle then I will have to collect my child/children and forfeit fees paid.
I confirm that I am the person with lawful authority and/or parental responsibility of the child/children on the Enrolment Form and the details provided for my child/children are all true and correct and I will immediately inform Mumma’s Little Secret in the event of any change to this information. I understand it is my responsibility to ensure Mumma’s Little Secret has the most current and relevant information on my child/children.
I agree that I must provide more than 24 hours notice to change a booking or I will forfeit fees paid for my booking.
I understand that I am to provide current information (as required within enrolment form) in regards to any of my child/children’s medical conditions, including allergies and sensitivities, to Mumma’s Little Secret on enrolment.
Mumma’s little Secret reserves the right to refuse entry to any of its services to anyone for any reason and at any time.
I consent to my child/children being collected from or brought to the Centre by any person listed in the Authorised Person’s section of the Enrolment Form.
I understand that Mumma’s Little Secret will require any person collecting the child/children who is different to the person that dropped the child/children off to provide photo identification for matching to the Authorised Person’s section of the Enrolment Form prior to my child/children being released to them.
I accept that a decision made by Mumma’s Little Secret that my child/children is contagious or too ill to attend Mumma’s Little Secret is final and I agree to ensure my child/children is collected promptly after being informed of such a decision.
Mumma’s Little Secret does not take any responsibility for lost, stolen or damaged possessions.
For the protection of children with allergies, Mumma’s Little Secret does not allow food or drink to be brought into our service.
Mumma’s Little Secret does not provide feeding services.
Mumma’s Little Secret does not provide nappy changing services.
Mumma’s Little Secret does not provide pram minding facilities.
A Mumma’s Little Secret staff member can take a child/children to the closest women’s Westfield toilet if parents/guardians have given prior permission to do so.
I acknowledge that, Mumma’s Little Secret staff will at all times supervise all children to the best of their ability, however if an accident was to occur Mumma’s Little secret cannot be held liable.
